I'm looking to make a survey in order to get some imformation to help me decide if I should make a site that I've had in mind for a while now.

What I want to do is have a form that spans several pages, then at the end to either insert the answers into a mySQL database or to email them to me and I will put the answers in Excel (I think the PHP/mySQL way will be quicker), but I'm having trouble finding a script to help.

What is the best way to have a multiple page form, or (and even better) does anyone know of any free scripts that you can download that make multi page forms for you (like EventHandler, but free).
